Help Us Make TXHES Work for You

We need your perspective and hope that you will take 10 minutes to complete the confidential online survey, https://bit.ly/TXHES_Survey. By doing so, you will be providing TXHES with valuable information that will improve services for you.

The quality improvement project is being conducted by Angela Scoggins, Doctoral Candidate Vanderbilt Peabody College. You will begin by answering a series of questions about TXHES’ website, communication, inclusion, and programming. Confidentiality of your responses will be strictly maintained.

If you choose to add your name and email address, you will automatically be entered in a random drawing for a $100.00 Amazon Gift Card. Winners will be notified by email and provided with information about how to collect the prize.

For questions about the content of the questionnaire or technical questions about the survey, please contact angela.t.scoggins@vanderbilt.edu

Thank you for your participation!
